I authorize my child to participate in the five-week Young Scribe's Literacy Program (YSLP).

Consent to be photographed: Throughout the program, we will take pictures of the children involved and we may use these pictures for promotional purposes only to promote our Young Scribe's Literacy Program (YSLP) and to promote our various programs and offerings.

I authorize The Scribe's Institute to use my child's picture to promote its causes and promote its programs in print, online, and through television.

I consent to my child using computers, tablets, or other technology as part of the program.
